Ishizu Ishtar (voiced by Karen Neill) is Marik Ishtar's older sister and the owner of the Millennium Necklace. She is Marik's Older Sister, and Odion's younger sister. She is one of Yugi's guardians who guards the pharaoh's tomb, and with her Millennium Necklace, has the power to see the past, and predict the future. She uses an Egyptian Fairy Deck, which specializes in having the opponent run out of the cards, and lose the duel.

- Ishizu will first appear in The Next King of Games.
- Ishizu will guest star in From Egypt with Love, and will fall in love with Patrick Dragonheart.
- Ishizu will guest star in Jeffrey, Jaden & Friends' Storm Adventures of Avatar. In it, Patrick will propose to Ishizu, to which Ishizu will happily say "yes."
- Ishizu will finally marry Patrick in Millennium Marriage.
- Ishizu will appear again in The 8th Millennium Item!
- In Jeffrey, Jaden, Hiccup & Friends' Storm Adventures of Avengers: Infinity War, Ishizu uses the Millennium Necklace to foresee the 14,000,605 futures involving the battle against Thanos, including the one where they win. In the end, she ends up as a victim of Thanos' snap.
